The form also asks for the requester’s name, address, email, phone, signature, and date. Internal fields show the actual Soda Springs workflow: assignment to a custodian, attorney review, full or partial approval, denial, an extension letter, an estimate, a final bill, and a release date. No official arrest blotter or public police case search was found. That absence matters: use the form and phone rather than an unofficial name-search site.
